Athletics Forms
In order to be cleared for participation in sports, a combination of forms from The Department of Education & Saint Joseph High School must be completed annually before an athlete can be involved in any athletic activity, including tryouts/practice. The required forms must be read and signed by the Parent/Guardian and the Student.
- 2026 Football Sport Paperwork is due on Monday, July 13
- 2026 Fall Sports Paperwork is due Monday, August 3
- 2026-2027 Winter Sports Paperwork is due on Monday, November 2
- 2027 Spring Sports Paperwork is due on Monday, February 22
When you submit a sports packet, it will be sent directly to the Saint Joseph High School Athletic Trainer. The Athletic Trainer will review all parts of the athletic sports packet to make sure everything is completed and signed by the parent/guardian and the student. A new athletics sports packet is required for every school year. In order to be eligible for the first day of tryouts, the completed packet must be submitted to the school Athletic Trainer according to the above deadlines. Forms not submitted within the specified timeframe may result in the prospective student-athlete notbeing eligible for the start of tryouts. Coaches will be notified of any ineligible players.
Please note:
If your son has ASTHMA, they must hand in the Asthma Action Plan from your doctor every school year. Asthma Action Plan

First Sport of the School Year - This is Needed
If this is the FIRST sport you are participating in during the current school year an entire Sports Packet must be submitted. The physical portion from pages 2-6 will be taken and completed by your primary care physician. The following pages will be returned to the Athletic Trainer: pages 6-11, 13, & 17-19.
Three Months Since Your Last Physical - This is Needed
If your Current Physical is Over 3 Months Old but is still Valid, a Health History Update must be submitted for Every Season during the school year.
